Candidates who submit an incident report following an in-centre exam incident will now automatically receive a copy of their completed form by email.
Acting on feedback from our September session, candidates will now receive a clear and easy-to-read summary of the information submitted on their exam centre incident form.
This means that candidates will have their own record of any incident report they make, which can be used to support any subsequent mitigating circumstances application.
Reporting an incident report in an exam centre
If you are sitting your exam(s) in an exam centre, and you experience issues during your exam, please inform the examination invigilator as soon as the incident occurs.
To complete an incident report after your exam:
Please make sure you provide the correct email address on the form, as this will be the address we will send the copy of your incident report to.
Please note that the incident report can be used as supporting evidence for a mitigating circumstance application; the form does not replace a mitigating circumstances application.
Amending an incident report
If you review your incident report summary, and wish to amend the information, please submit a new incident report form. Your new form should be submitted no more than 24 hours after you submitted your original form.
You can access the incident report form using the link in your exam upload confirmation email. Please do not reply directly to the incident report summary email.
